All residents entering residential homes within their respective communities should be entitled to a personalised individual care plan. The plan should be based on a thorough holistic assessment of the individual’s needs and preferences, as well as their abilities and aspirations.

Care plans for residents should, wherever possible, be developed in partnership with the resident, include communication with the family or next of kin, and with the necessary professionals where this is applicable.
