All medications that are administered to individuals are required to have specific administration instructions. This can often be an issue and is often picked up during internal and external audits or sadly if there is an error. If ‘as directed’ labelled medications are found within a service we recommend that an interim risk assessment and risk management plan is put in place to mitigate the risk and to demonstrate that the service has identified the issue and is working towards resolution.
