Transfers and discharges may occur for a variety of reasons, including changes in a resident’s health needs, hospital admission, return to their own home, relocation to another setting, or end-of-life care arrangements.

This policy sets out the principles and procedures for the safe, timely and person-centred transfer and discharge of residents.

It aims to ensure that any move, whether temporary or permanent, is carefully planned, clearly communicated, and carried out in a way that protects the health, safety, dignity and well-being of each resident.
